Good Home Community Living

Home and Community-Based Services (HCS) Program

 

Services Options

 

Persons enrolled in Good Home Community Living’s HCS program, may receive the following services, based on individual needs and eligibility:

 

·       Residential Assistance: includes the provision of assistance and support necessary for the consumer to:

·       complete personal care, health maintenance, and independent living tasks

·       participate in community activities

·       develop, retain and improve community living skills

 

Residential Assignment

Own/Family Home

Supported Home Living: provided to individuals residing in their own residence or the residence of their natural or adoptive family; or to individuals receiving foster care services from the Teas Department of Protective and Regulatory Services.

 

 

 

HCS Foster/Companion Care Home

HCS Foster/Companion Care: provided to consumers living in the home of a foster care provider or with a companion care provider. Consumers receiving Foster Care Services are not eligible for Respite Services. 

 

Residential Support

Shift Staffed Residence

 

 

 

 

 

 

Supervised Living

 

 

 

 

Residential Support:  provided to consumers living in a setting in which services are provided by employees assigned on a scheduled shift basis.

 

 

Supervised Living: is provided in a residence in which the HCS program provider holds a property interest. Supervised Living is provided by residential assistance providers who provide services and supports as needed by individuals and who are present in the residence during normal sleeping hours.

 

 

Residential assistance includes the following:

    • Providing direct personal assistance with activities of daily living (grooming, eating bathing, dressing, and personal hygiene)
    • Assisting with meal planning and preparation
    • Securing and providing transportation
    • Assisting with housekeeping
    • Assisting with ambulation and mobility
    • Reinforcing counseling and therapy activities
    • Assisting with medications and the performance of tasks delegated by a Registered Nurse
    • Supervising individuals’ safety and security
    • Facilitating inclusion in community activities, use of natural supports, social interaction, participation in leisure activities, and development of socially valued behaviors
    • Providing habilitation, exclusive of Day Habilitation

 

·       Case Management:  provided to each consumer by a single, identified HCS provider staff person for the purpose of:

·       coordinating the development and implementation of a consumer’s Individual Plan of Care and Individual Service Plan

·       coordinating and monitoring the delivery of services

·       arranging for transportation

·       planning and arranging for a consumer’s discharge or transfer

·       Integrating various aspects of services delivered under the waiver and through other sources

·       Recording each person’s progress

·       Pre-discharge plan development. 

 

·       Day Habilitation:  includes age-appropriate activities designed to assist in acquisition, retention, and/or improvement in adaptive skills necessary for continued community residence.

Day habilitation includes:

  • Assisting individuals in acquiring, retaining, and/or improving self-help, socialization, and adaptive skills necessary to reside successfully in the community.
  • Providing individuals with relevant activities that enhance self-esteem and maximize functional level.
  • Complementing any counseling and therapies listed in the IPC.
  • Reinforcing skills or lessons taught in school, therapy, or other settings.
  • Providing training and support activities which promote the individual’s integration and participation in the community.
  • Providing assistance for individuals whom cannot manage their personal care needs during day habilitation activities.
  • Providing transportation necessary for the individual’s participation in day habilitation activities.
  • Providing services which are not being provided under Section 110 of the Rehabilitation Act of 1973 or Section 602(16) and (17) of the Individuals with Disabilities Education Act.

·       Supported Employment:  includes training and supervision provided to consumers with a documented/identified need and desire for employment and who are employed in an integrated work setting outside of the consumer’s residence.

 

    • Supported employment is provided in an integrated provision of ongoing individualized support services needed to sustain paid work by the individual, including supervision and training.
    • The individuals receiving supported employment are compensated by the employer in accordance with the Fair Labor Standards Act.
    • Supported employment is provided when these services are not available or funded through the state education agency or the state rehabilitation agency.

·       Nursing Services:  includes the performance of health care procedures and monitoring an individual’s health condition by Registered Nurses or Licensed Vocational Nurses.

    • Administering medication
    • Monitoring the individual’s use of medication
    • Monitoring health data and information
    • Assisting the individual and/or LAR to secure emergency medical services for the individual
    • Making referrals for appropriate medical services
    • Performing health care procedures ordered or prescribed by a physician/medical practitioner and required by standards of professional practice or law to be performed by licensed nursing personnel
    • Delegating and monitoring tasks assigned to other service providers by a registered nurse (RN) in accordance with state law.

·       Counseling and Therapies:  includes counseling, diagnostic and therapeutic activities, based on individual need for:

·       Audiology

·       Speech

·       Occupational Therapy

·       Physical Therapy

·       Dietary

·       Social Work

·       Psychology

 

·       Dental Treatment:  include specific emergency dental treatment, preventive dental treatment, therapeutic and orthodontic dental treatments. Dental treatment covers up to $1000.00 per IPC year.

·       Respite Care:  includes planned or emergency care for a consumer when the person normally responsible for providing care is unable to do so.  Respite is available to consumers who are living in their family home and who have a residential assignment of Own/Family Home. Eligible individuals can receive 30 days or 300 hours of respite per IPC year.

 

Respite includes:

    • Assistance with training in self-help and independent living skills
    • Provision of room and board when provided overnight in a setting other than the person’s normal residence
    • Support of individuals in the HCS Program who are eligible for respite and in need of emergency or planned short-term care
    • Assistance with receiving other needed services
    • Assistance with securing and providing transportation

Adaptive Aids: includes devices, controls or appliances which enable persons with mental retardation to retain or increase their abilities to perform activities of daily living or control the environment in which they live.  Adaptive Aids may also include medically necessary supplies and medications. Adaptive Aids covers up to $10,000.00 per IPC year.

Adaptive aids consist of the following services, including repair and maintenance not covered by the warranty: the full range of lifts, mobility aids, control switches/pneumatic switches and devices, environmental control units, medically necessary supplies, and communication aids.

 

·       Minor Home Modifications:  includes assessing the need for, arranging for and providing modifications and/or improvement to a consumer’s living quarters to allow for community living and ensure safety, security and accessibility. Minor Home Modifications covers up to $7,500.00 for a lifetime and $300.00 annually there after.

    • Purchase and repair of wheelchair ramps
    • Modification to bathroom facilities
    • Modifications to kitchen facilities
    • Specialized accessibility and safety adaptations, including repair and maintenance

Services: The Home and Community-Based (HCS) service array is broad and has been established by the Texas State Medicaid Waiver. The service array includes the following: Case Management, Respite Service, Supported Home Living, Foster Care, Residential Support, Social Work Service, Occupational Therapy, Physical Therapy, Speech Services, Audiology Services, Psychology Services, Nursing Services, Dietary Services, Adaptive Equipment ($10,000/year), Minor Home Modification ($7,500 lifetime/$300 annual maintenance) and Dental Services ($1,000) per year, Day Habilitation of Supported Employment ($3,000/year).

Populations Served: Good Home Community Living HCS program does not target any particular population beyond the program eligibility criteria. Good Home Community Living has a zero reject policy. We enroll anyone who chooses our program. We work hard for the consumer in meeting all of their daily living needs.

Admission Criteria: The applicant for HCS must be eligible for Medicaid coverage. Other requirements include assignment of a level of care by the Texas Department of Health and a determination of mental retardation (DMR) or Diagnosis and Evaluation (D & E) prior to the proposed admission. The projected service needs must also be within the established ceiling cost for HCS services. Eligible individuals must reside in the State of Texas to receive services through Good Home Community Living HCS Program.

 

Good Home Community Living HCS Program offers multiple community based services and uses generic community resources in order to assist individuals with mental retardation to reside successfully in the community. Each individual receives encouragement to reach their full cognitive, social, and employment potential through training and participation in a normalized home environment as well as regular community venues. Good Home Community Living HCS Program is organized to provide oversight of all service providers in order to ensure that quality services and supports are provided to each individual enrolled in the program as authorized in the Individual Plan of Care and to assist each individual in achieving the outcomes identified in the Individual Service Plan. This HCS program will provide services to all eligible individuals who select Good Home Community Living as their HCS provider.
